How we create an individual EHC Plan (Education, Health, Care Plan)

The aim of our provision is to identify and assess the SEND and to plan the provision needed for each pupil to achieve his or her best at Becontree.

 

Some SEND pupil’s may need extra support to achieve their learning potential. If this is the case this is what we will do:

 

 

 

SEND Procedures

Some SEND pupil’s may need support from professionals outside of the school setting to achieve their potential. In order to do this the school pays for Educational Inclusion Advisors and seeks advice from specialists in specific areas. Some examples are:

  • Behaviour Advisors
  • Speech and Language Advisors
  • Educational Psychologist
  • Autism (ASD) advisors

 

One Plan

 

One plan will replace the Statement of Special Educational Needs. This is called the Education, Health and Care (EHC) Plan. It will cover all areas of special educational provision, health provision and social care provision.

 

Barking and Dagenham SEN department will be responsible for the EHC Plan and reviews will be held once a year to make sure that the provision in place is still appropriate.

 

Common Assessment Form

 

In order to help your son/daughter achieve their learning potential we may ask your permission to fill in a Common Assessment Form (CAF). Either the SENCo, Deputy Head or Parent Support Advisor will ask you to attend a meeting to provide some information about your child and your family and in your presence we will fill in the CAF. We will only include information that you tell the member of staff. The CAF will provide an overview of your family and will help us to decide together what we need to organise next. The CAF is a confidential document and will not be shared with anyone unless you have given your permission to do so.

 

Families who have previously had CAF’s have found this to be a supportive process as they have found that the agreed referral to the appropriate agencies has helped them to support their child to be successful at Becontree.
